General forms

Account amendment - if you have moved address you will need to let us know. Please fill in this form and send it into us.

Power of Attorney form -if you are applying for a new account with a Power of Attorney or need to add a Power of Attorney to an existing account. Please fill in this form and send it into us.

Court of Protection Order application form - if you are applying for a new account with a Court of Protection or need to add a Court of Protection to an existing account. Please fill in this form and send it into us.

Savings forms

Lost/Stolen passbook - if you are unable to find your passbook whether it being lost or stolen you will need to print and complete this form and attend your local branch with the required identification for a new passbook to be issued.

Add/Delete account holder – if you are looking to add or remove an account holder please use this form.

Statutory Declaration form – the form you will need if the total value of the estate of a deceased customer is less than £15,000.

ISA transfer forms

Cash ISA to Cash ISA transfer – the form you will need to transfer your Cash ISA from another provider to a West Brom Cash ISA.

Stocks and Shares ISA to Cash ISA transfer – the form you will need to transfer your Stocks and Shares ISA from another provider to a West Brom Cash ISA.

Cash ISA to Cash ISA transfer (For WeBSave accounts) – the form you will need to transfer your Cash ISA from another provider to a WeBSave Cash ISA.

Stocks and Shares ISA to Cash ISA transfer (For WeBSave accounts) – the form you will need to transfer your Stocks and Shares ISA from another provider to a WeBSave Cash ISA.

Mortgage forms

Income and expenditure form – the form you will need to complete when looking to make a change to your mortgage.

Direct Debit instruction – the form you will need if you wish to change your bank details as to where we take the mortgage payment from or if you wish to instate or reinstate a direct debit.
