Account closure form - below £30,000
This is the form you’ll need to complete if the total balance held in the account(s) of the person who passed away is below £30,000. Where the total value of the account(s) is over £5,000, the form must also be witnessed by a solicitor, Commissioner for Oaths or Authorised County Court Official.

This is the form you’ll need to complete if the total balance held in the account(s) of the person who passed away is above £30,000 or where Grant of Probate/Letters of Administration has been obtained, regardless of the balance.
